Program Forms Accordion Closed
All graduate students start out in the comprehensive examination plan. Students completing their degree in comprehensive examination plans can find department forms here: M.S. Mathematics or M.S. StatisticsÌý²¹²Ô»åÌýM.S. Mathematics Education. See M.S. Mathematics Education portfolio directions. Students in the MS in Mathematics program may apply to change to the Thesis plan.  The Office of Graduate and Professional Studies has checklists of requirements for theses.

Travel Accordion Closed
Before starting any trip, please request approval from the chair and financial oversight analyst in an email and include the following:
- Dates of Travel
- Purpose of travel
- Total cost of trip to include meals, transportation, registration, and lodging
- How much will be covered by a third party
- How much are you asking the department to pay
If travel email is approved,  submit an Employee Service Center request .
Complete the Annual Travel Certification, NAU Policy training, and NAU Authorized driver training if driving during travel .
An itinerary  is required to be uploaded upon return from the business trip.
